Invoice templates

However, I am looking into doing some hauls for other brokers and they need you to send in an 'INVOICE' So, could someone please send me a copy via my email of how you do you invoicing? I'm looking for a scanned example of the actual invoice... You could blot out your name and important information... I am new to the hopper business and would like someones professional aproach as to how you invoice and then I could model mine after yours... I haven't had to actually do any invoicing myself because it's not necessary with the broker that I use.
